Clinic Students
Former Gittis Legal Clinics students may request that their faculty supervisor complete a Form Affirmation as to Applicant’s Law-Related Employment and/or Solo Practice for New York Bar Admission by doing the following.
- Complete the top section: your name, the Appellate Division, and your BOLE ID #.
- Email clinic@law.upenn.edu with “NY Bar Employment Affirmation” in the subject line. Attach your Affidavit form to the e-mail. In your e-mail please include:

- The name of the clinic(s) you were enrolled in. [Note: All currently enrolled clinic students should submit this request after the end of the semester’s examination period.)
- The semester/year you were enrolled and if you were an advanced student.
- The name of your faculty supervisor.
- The address where you would like the completed form to be mailed to (or advise if you prefer to pick the form up in our office).
We will complete the Affirmation and email it to you as digitally signed PDF. The New York Bar continues to accept digital forms.
Normal processing time is 1-2 weeks. Please do not wait until the last minute to submit these forms as they may not be returned promptly if your faculty supervisor is not currently in the office or is otherwise unavailable when you make the request.
Former Gittis Legal Clinics students may request that their faculty supervisor complete an Affidavit related to the New York Pro Bono Requirement by doing the following.
- Complete the first section with your name, address, and BOLE ID number. We will complete the rest of the first page for you.
- Email clinic@law.upenn.edu with “NY Pro Bono Requirement Affidavit” in the subject line and attach your Affidavit form. Include in your email your former faculty supervisor’s name, the clinic in which you were enrolled, and the semester/year. [Note: Students currently enrolled in a clinical course should not request this until after the end of that semester’s examination period.]
- After we complete the first page, we will return the entire form via email for you to sign before a notary. You can choose to get the form notarized on your own, or you may book an in-person notary appointment with the Clinic’s Litigation Paralegal, Andrea Festa, at this Bookings link. Andrea’s office is located at S-G35. When booking, please be sure to select the first option, “In-Person Notarization with Andrea.” Please also remember to bring with you a valid form of identification, such as a driver’s license or passport. Note that electronic notarizations are not available.
- Leave the form with Andrea, or email the signed and notarized form back to the Gittis Legal Clinics office (clinic@law.upenn.edu) for the faculty supervisor to sign the Supervisor Certification on page 3. [The New York Bar requires the faculty supervisor to sign after the applicant signs.]
After the faculty supervisor digitally signs the certification page, we will email the completed Affidavit to you as a PDF. The New York Bar continues to accept digital forms.

Externship Students
Externship students should contact their placement site supervisor directly to complete New York Bar Law-Related Employment Affidavits or Pro Bono Requirement Affidavits.
