Former Clinic students may request that their Faculty Supervisor complete a Law-Related Employment Affidavit for New York Bar Admission by doing the following:
E-mail Joyce Poole (joycep@law.upenn.edu) and cc: clinic@law.upenn.edu with “NY Bar Employment Affidavit” in the subject line. Attach your Affidavit form to the e-mail. You will only need to complete the top section which includes your name, the Appellate Division and your BOLE ID #. In your e-mail please include:
- The name of the Clinic(s) you were enrolled in. (Note that all currently enrolled Clinic students should only submit this request after the end of the semester’s examination period).
- What semester/year you were enrolled and if you were an Advanced student.
- Name of your Faculty Supervisor.
- Address where you would like the completed form to be mailed to (or advise if you prefer to pick the form up in our office). In addition to mailing the Affidavit to you, we will also e-mail you a pdf of the completed form.

Normal processing time is approximately 2 and a half weeks - please do not wait until the last minute to submit these forms as they may not be returned promptly if your Faculty Supervisor is not currently in the office or is otherwise unavailable when you make the request.
- Note: Externship students should contact their placement site Supervisor directly to complete this form.
Former Clinic students may request that their Faculty Supervisor complete an Affidavit related to the New York Pro Bono Requirement by doing the following:
Page 2 of the Application for Admission to Practice as an Attorney and Counselor-at-Law in the State of New York requires the applicant to get their signature notarized. You must complete this step before contacting the Clinic office to request the Faculty Supervisor’s certification.
After you have gotten your signature notarized, e-mail Joyce Poole (joycep@law.upenn.edu) and cc: clinic@law.upenn.edu with “NY Pro Bono Requirement Affidavit” in the subject line. Include in your e-mail your former Faculty Supervisor’s name, which clinic you were enrolled in and during which semester/year. Attach your Affidavit form to the e-mail. You will only need to complete the first section with your name and address and we will complete the rest of the form for you. (Note that students currently enrolled in a clinical course should not request this until after the end of that semester’s examination period).
